When Is PMMA on an iBar the Right Choice?
The most representative indication is a patient receiving full-arch restorations in both arches, where the opposing arch is restored with full-zirconia ceramics.
When chewing forces act between a rigid arch (zirconia) and an equally rigid opposing arch (zirconia), the entire load transmits directly to the implants—with no shock-absorbing mechanism in between. PMMA, with its inherent elasticity, functions as a “biological shock absorber”: absorbing part of the chewing load and reducing peak forces transmitted to the bone surrounding the implants and to the abutment connections. From a biomechanical standpoint, this is a clinically valuable rationale—not merely an economic one.
In addition, the fabrication cost of PMMA on an iBar is lower than other full-arch implant restorations, opening access to a wider range of patients without compromising on core quality.
Considerations
Shade matching is a factor that requires careful consideration. The current PMMA multi-layer shade system is built on the basic VITA shade scale—suitable for the vast majority of clinical cases, but a certain degree of shade mismatch may occur when paired with a highly customised zirconia restoration on the opposing arch. This is information that must be discussed clearly with the patient before the treatment plan is finalised, to avoid mismanaged expectations.
